FAST Sports Waiver, Release, and Consent Form
This form covers the FAST Sports After School Program (including Day Camp on no-school days) and FAST Sports Summer Camp (together, the "Program"). A parent or legal guardian must accept it online before a child can take part.
1. Assumption of Risk
I understand the Program includes physical activity and sports, including flag football, conditioning, speed and agility work, and other games. These activities carry real risk of injury.
Risks include, but are not limited to:
- Sprains, strains, broken bones, cuts, and bruises
- Collisions with other players, equipment, or the ground
- Concussion or other head injury
- Heat illness, dehydration, sunburn, insect bites, and weather-related injury
- Injury during off-site trips, games, or travel to and from activities
I understand these risks can happen even when staff are careful and rules are followed. I choose to let my child take part and accept these risks on my child's behalf.
2. Release and Hold Harmless
To the fullest extent allowed by North Carolina law, I release FAST Sports, its owners, coaches, staff, volunteers, and partner facilities (the "Released Parties") from claims for injury, illness, or property loss arising from my child's participation in the Program.
I agree not to sue the Released Parties for those claims, and to hold them harmless from claims brought on my child's behalf.
This release does not cover gross negligence or willful misconduct.
3. Medical Authorization
My child is physically able to take part in sports activities. I will tell FAST Sports about any change in my child's health.
If my child is injured or ill and I can't be reached, I authorize Program staff to give basic first aid and to get emergency medical care, including transport to a hospital. I am responsible for any medical costs.
4. Pickup, Media, and Conduct
Authorized pickup. Only the people listed on this registration may pick up my child. Staff may ask for photo ID.
Photo and video. FAST Sports may use photos and video of my child taken during the Program on its website, social media, and promotional materials, without payment, unless I opt out below.
Conduct. My child will follow staff instructions and treat others with respect. FAST Sports may remove a child from an activity or the Program for unsafe or disruptive behavior. Refunds follow the program policy shown on this registration.
5. Electronic Agreement
I am the parent or legal guardian of the participant named above. By checking the box and typing my name below, I agree this is my electronic signature and has the same effect as a handwritten one. This agreement stays in effect for all Program sessions in the 2026–27 school year and Summer 2027 unless I revoke it in writing.
